Hard to tell from the photo, but I’m guessing the muzzle is also the pommel, meaning that if the knife is pointing at an opponent, the gun is pointing (approximately) at its owner. Lovely.
Shoot first, then if that doesn’t work and a pipsqueak .22 probably won’t, resort to the knife. Otherwise there might be a surprise in store for someone. I’m thinking of these examples of table-cutlery with built-in flintlocks, presumably used by the more aggressive restaurant critic, but which also have the guns pointed the wrong way…

Spikes on knuckle-guards aren’t that unusual - a well-known example is the “Night Stalker” prop from the Sylvester Stallone thriller “Cobra”…

…but genuine WW1 trench knives had them as well…

…sometimes mounted on an actual knuckleduster…


The OP knife with its built-in zip-gun is described as “improvised”, but there have also been “official” gunblades - the Elgin pistol with knife built on…

…and various modern examples like the NRS - recoil is supposedly very low but still, does this look safe to you?…

… both of which actually have all the dangerous bits pointing away from the weapon’s owner. Quite an improvement…
