Photograph caption dated March 1, 1927 reads, “Authorities today were investigating the death of John Lee O'Brien, 19-year-old Los Angeles youth when he fell from an Ocean Park roller coaster into the sea. The photo-diagram shows the point at which O'Brien fell from the coaster car and the dotted line indicates the path of his plunge of 50 feet into the ocean to his death.“

God yes, finally! I’ve recently found a couple of examples in Canadian newspapers too and I was flabbergasted when I first saw them. Here’s one from 1947, via the Toronto Star:
“Diagram shows fall taken by Joseph Gariepy, 72, who toppled off iron bar at the Casselman, Ont., Roman Catholic Church yesterday. Elderly building painter fell 90 feet to his death.
“
And another, undated, again from the Star, and this time there does seem to be a crowd gathered directly where he fell (either coincidence or taken not long after):
“Falls five storeys to death. Morris Powlatsky; aged 53; of Winnipeg; a guest at the Walker House; fell from the fire escape on the fifth floor of the Walker House; striking on his head on the sidewalk being killed instantly. His son; who was in the city with him; said his father had been in poor health. No inquest will be held.”
